A fluid balance chart is a 24-hour clinical document used to track a patient’s total fluid intake and output, crucial for preventing overload or dehydration. It records oral, intravenous, and food intake against urine, vomit, and drain losses. 

The formula is: 

Fluid Balance = Total Intake (mL)  - Total Output (mL) 

Check a filled fluid chart below, but in your CBT exam  fluid input and out put section already filled, your task is to find out Balance from  total fluids input and output.

Equation: Total Input - Total output 

950 - 505 = 445, add '+' to make it positive balance 

Answer= + 445 mL
